🟢 A — Actions Things that need to happen. Assigned. Tracked. Completed. No orphaned tasks.
🟡 I — Issues Things that have gone wrong. Don't bury them — resolve them.
🔵 D — Decisions Choices made with clear rationale and authority. Documented. Not forgotten.
